For your convenience, you may make your contributions to Saint Luke Parish by MasterCard, Visa, Discover or American Express
Credit Cards.  (Sorry, we are not equipped to accept debit cards.)

Two forms are required, one from each group:

(1) Annual Authorization Form
     by which you give us your account information
     Choose either:   CREDIT CARD    or     DIRECT DEBIT

(2) Annual Giving Chart
     by which you instruct us as to the amount of each contribution. 
     Choose as above:   CREDIT CARD     or     DIRECT DEBIT

These may be downloaded in PDF format by clicking on the form titles above.  Once you've filled out the forms, please return them to us by mail or in any collection.  Each November, credit card contributors will be sent a new set of forms for the coming year.
